Written Requests for Birth or Death Certificates

Written requests must include acceptable identification and the following information:

Birth Certificates

  1. Name of child at birth
  2. Date of birth
  3. Place of birth (city, county, hospital)
  4. Full maiden name of mother
  5. Birthplace state and birth date of mother
  6. Full name of father
  7. Birthplace state and birth date of father
  8. Relationship of requestor to person
  9. Signature of requestor
  10. Picture identification
  11. Mailing address
  12. Payment (payable to Davis County Vital Records)

Death Certificates

  1. Full name of deceased
  2. Date of death
  3. Place of death (city, county)
  4. Full name of father
  5. Full maiden name of mother
  6. Name of spouse if deceased was married
  7. Relationship of requestor to deceased
  8. Reason for requesting certificate
  9. Signature of requestor
  10. Picture identification
  11. Mailing address
  12. Payment (payable to Davis County Vital Records)
