The Alternatives Program

The Alternatives Program provides in-home services for medically fragile adults who qualify, and are at risk for entering a nursing home. The goal of the program is to keep frail adults in their own homes by providing and coordinating a variety of services. Clients' needs are assessed by a social worker who develops a care plan to address the needs identified.

Eligibility

  • Adults 18 years of age or older
  • Medically Fragile
  • Low income (100% of Poverty)
  • Asset Limitations (Less than $6,000 for a family of one, or $12,000 for a family of two)
  • Medical deductions apply
  • Nominal fee based on a sliding fee schedule (Generally $42 a month or less)
