Jensen 800 West State Street PO Box 618 Farmington UT 84025 801-451-4488 801-451-4482 When a defendant is required to complete community service, the court expects the defendant to locate his or her own community service source. The following is a list of some of the providers which the court approves, but these providers are not obligated to assist our defendants. This list is always subject to change WITHOUT advance notice. The defendant is expected to CALL providers FIRST to make arrangement for community service. Please understand some of these providers are not available all year as their work is seasonal. Please treat the providers with respect and courtesy at all times. If the court receives word that the Defendant IN ANY WAY failed to comply or cooperate with a provider, the Defendant will immediately be held in contempt and subject to additional fines and/or penalties including denial of any or all community service hours completed to date. IT IS THE DEFENDANT’S RESPONSIBILITY TO RETURN THE COMMUNITY SERVICE DOCUMENTS TO THE COURT The court will require the name, address and phone number of the provider the defendant works with, along with the supervisor’s name. The supervisor will be required to sign off all hour worked and briefly describe the type of work done. The defendant must return the service form to the court for full credit. Do not ask or expect the provider or supervisor to do this.
