Home & Community Based Alternatives Program
What is The Alternatives Program (TAP)
The Alternatives Program provides in-home care for medically fragile adults
who qualify, and are at risk for entering a nursing home. The goal of
the program is to keep frail adults in their own homes by providing and
coordinating a variety of services. The clients' needs are assessed by
a social worker and a Registered Nurse that develops a care plan to address
the needs identified.
Who is eligible?
- Adults 18 years of age or older
- Medically Fragile
- Low income (100% of Poverty)
- Asset Limitations (Less than $6,000 for family of one or $12,000 for family of two)
- Medical deductions apply
- Nominal fee based on a sliding fee schedule (Generally $39.00 a month or less)
