Passport Information
Items required
- Completed application – Applications take about 20 minutes to complete
- Certified copy of Birth Certificate – previous passport – naturalization papers
(These are submitted to the passport office but are returned with your passport – copies are not acceptable)
- Social Security #
- 2 passport photos -- (taken within the last six months) 2x2 inches white background
- Proof of identity – valid drivers license, government, military, or state ID, or previous passport
- Children under 16 applying for passports require both parents signatures. If the second parent is not available to sign, the appearing parent must:
Present evidence that he/she has
- sole legal custody of the child or
- notarized written consent of the other parent for the issuance of the passport or
- written statement explaining why non-applying parent's consent cannot be obtained.
- even if the child has a previous passport a birth certificate is still mandatory.
- All minors must appear in person when applying for a passport
Applications are accepted in our office Monday thru Friday from 1:00 p.m. to 4:00 p.m.

Fees
Fees: (All fees are regulated by the US Department of State, Passport Agency.)
The Davis County Clerk processing fee for handling passport applications is $25.00
per application.
In addition to fees charged by the county clerk, The US Department of State charges the
following fees for passport applications:

Applicants 16 and older
|
- $75.00 for regular processing (6-8 weeks)
- $135.00 for expedited processing (3-4 weeks)

Applicants 15 and younger
|
- $60.00 for regular processing (6-8 weeks)
- $120.00 for expedited processing (3-4 weeks)

The US Department of State fee is mailed with the application and therefore
must be in the form of a check or money order (cash cannot be mailed.)
