The Vital Records staff provides services to the public in obtaining birth and death certificates.
General Information
Birth certificates are available approximately 10 days after the child is born.
Birth certificates for births anywhere in the state of Utah that have occurred from 1953 to the present may be obtained here – with more years being added.
Utah County birth certificates are also available through this office back to 1905.
The small laminated birth card is no longer issued since many agencies do not consider it a legal document.
Births outside of Utah: call (801) 525-5150 or go to VitalChek. There is an additional charge for this service.
Death certificates should be obtained from the county where the death occurred or there is an additional $12 fee and 24 hours needed if obtained through this office.
Note:Marriage certificates must be obtained at Davis County Clerk/Auditor’s office, (801) 451-3218.
Certificate Requests
Birth or death certificate requests can be made either through the mail or in person.
Requests require the requestor to be the individual or an immediate family member (child, spouse, parent, sibling, grandparent, grandchild) of the person whose record is being requested.
