There are several programs and services including the Alternatives Program, Medicaid Aging Waiver Program, New Choices Waiver Program, and Veterans Directed Home and Community Based Services that may be able to help your aging loved one. To access these programs, contact Davis County Seniors Services at 801-525-5050. A case manager will discuss your mother's situation with you and make referrals to appropriate services. If necessary, a case manager will arrange to visit with you and your mother in person to perform a more in-depth assessment.
Age eligibility for our home-based programs varies by program. Please call a case manager at 801-525-5050 to learn about requirements for these programs.
Family Caregiver Support Program - Person receiving care - 60 years
Senior Activity Centers - 60 years
Meals on Wheels - 60 years
Most services are available on a 'suggested donation' basis. We do not deny services to someone if they are unable to make a donation. All donations are used to expand programs and additional services. 

Some home-based services charge a fee based on a sliding scale that is tied to level of income, or individuals may be required to 'spend down' their income in order to be eligible for assistance. Please call a case manager at 801-525-5050 to discuss eligibility and options.
