Targeted case management is a service that assists Davis County families starting at child birth and through age 8 to gain access to needed medical, social, educational, and other services. Services are available to all families regardless of income or insurance coverage.
The overall goal of the service is not only to help Medicaid recipients access
needed services, but to ensure that services are coordinated among all agencies and providers involved.
The home visitation program provides parents and caregivers with free and confidential in-home education and
one-on-one support. Your nurse wants you to have all the resources that you need to give your child a happy and
healthy start in life.
Your nurse will come to your home and provide information about:
- Child’s developmental milestones
- Child health and safety
- Parenting concerns and questions
- Postpartum care and depression screening
- Referrals to early intervention services
- Community resources
- Immunizations information
