Tier II Electronic Submission Form

Section 312 of EPCRA [40 CFR Part 370] requires that facilities submit Tier II Information for hazardous chemicals, which include EHSs. A Tier II Report is a means of submitting the required information to state or tribal and local planning and response agencies, which includes fire departments. Reports must include information on the chemicals present at the facility, facility information, and facility contacts.

The Davis County LPEC has provided this online form in order to upload electronic Tier II Submit files along with accompanying documents. The LEPC does NOT need a paper copy of the entire report if it is submitted in electronic format.
